If you want stars added, and not to replace all the other settings, you have to use the add_to_ mechanism(
wiki):
Code:
add_to_extra_titlepage_entries:,stars
add_to_include_subject_tags:,stars

Different save path? I have no idea what you mean in this context--you're in Calibre, it decides where to save based on your library...

If the epub doesn't need new chapters, by default FFF doesn't update. If you want to update just the Calibre metadata, change the 'If Story Already Exists in Anthology?' AKA 'Update Mode' to 'Update Calibre Metadata from Web Site'.
If you are just starting with FFF and/or Calibre and expect to want to use a bunch of custom columns, you should probably set up an FFF 'Saved Metadata Column':

Once that column is populated (by FFF download or update), you can use the 'Update Calibre Metadata from Saved Metadata Column' update mode to update other columns without needing to fetch from the sites again.
For FFF saved metadata, you want Column Type long text. Set the "Interpret as" to HTML if you want it to be easily readable, but don't try to edit what FFF puts in that column then--Calibre will change the formatting and it won't work anymore for FFF.
