That could be that the icon has been removed from the device toolbar. When a device is connected, calibre changes the toolbar to one that includes device actions. You can customise this in the preferences. Make sure the plugin is on it.

Otherwise, as @PeterT said, please run calibre in debug mode and post the debug log.
