Run calibre from Terminal.ap by typing the following command and pressing enter:

/Applications/calibre.app/Contents/MacOS/calibre


it should work. If it does it indicates the problem is in launch services on your mac. You will need to reset its cache, a bit of googling should give you the commands to do that.
