- Add Yes/No to all to most question_dialog boxes.

Most of the dialog boxes that ask you for a yes/no decision now have 'Yes to All' and 'No to All' buttons now.

These are the applicable dialogs:

Reject URL? - %s is on your Reject URL list
Remove Reject URL? - Remove URL from Reject List?
Are You an Adult?
Skip Anthology Story? - %s is in series that you have an anthology book for.
Change Story URL? - title by author is already in your library with a different source URL:
Download as New Book? - (from answering no to above)

Fun fact: it takes some doing, but it is actually possible to trigger all six of those in the same download.
