A few oddments come to mind. What OS and version of calibre is your server running on? If running under Linux, is this a distro version or did you install from Kovid Goyal's site. Have you tried opening the PDF with a dedicated PDF reader on your computer? Does the PDF have any text or is it a collection of images?

Perhaps this message should have been posted in the Server forum. Though a better explanation of what you meant by all devices might have helped. You don't seem to have a dedicated ereader since you list iOS/browser and your image seems to be from a browser under iOS on an iPhone XS Max.

And why for the love of any deity you care to believe in, did you use that massive image instead of attaching it to your message using the Attach Files tool so we would see a thumbnail and open the image from that. If you look at the Guidelines, I think that the maximum image size to be placed in a message is 600x600.
