3/ No, it's the actual eInk refresh black flash. That setting only applies when actual text is displayed (i.e., the reader, or dict/wikipedia popups for instance), not UI elements.
You haven't told us on which device you're using KOReader, but the fact that you actually see this option should imply that it is some kind of eInk device, so you should be somewhat familiar with the phenomenon.

1) I turned alternate status bar off but I noticed it removed the entire header and bar. I only want the chapter ticks removed from up there if its possible.
2) Why does Koreader do that? It's the first reader I've seen that disables italic styling when bold is enabled. I guess I'll reserve myself to just using non bolded font with an increased contrast value for now. Unless there is a fix for this.
3) Ah, I'm a bit bugged to hear that. But no big deal.
I am using two android devices. The first one is an Alcatel Tetra and the other one is a Nextbook Premium 8th edition. The first one is a phone, the second one is a tablet. Both seem to work good with Koreader, except for the aforementioned issues.
