That looks like your entire user interface font for the viewer is monospaced. What is the interface font for calibre set to in the calibre preferences.

It's nice that you dont think they are expensive, I disagree. And you are proposing they be implemented for every annotation and then if they are implemented for every annotation, that means something else will need to be implemented to indicate an annotation has notes. I'm afraid, having lots of contiguous annotations in the same color, is not a common use case and I am not either uglifying or adding dots to them for it.
