An actual look at KOReader's crash.log would be helpful, but it simply sounds like your internal storage is corrupted (or got corrupted during the update/reinstall).
Was it always ejected properly, or did someone unplug it like a savage recently?
Do a filesystem check (then a database check in Calibre+Kobo Utilities for good measure), and then reinstall the latest OCP, making sure you *do* eject it properly this time
