I cannot see in the xHTML that each chapter reference item has been tagged by a number e.g. bibItem-54

Well, those two items--the chapter body link and the actual reference--have nothing in common. The id for the link, the "m-ISBN-chapter-1#m-ISBN-chapter-1-bibItem-54" is not the name of the reference, so...how does your initial link know where to go?

May I ask how you got this far? What did you do, to get this ePUB?
