Multiple authors - how does Calibre decide the folder structure?

Mostly out of curiosity. I don't intend to go tampering around with the folder structure.

When fixing up the metadata of books in PDF format, when I add an extra author sometimes I have to close the file because Calibre wants to move the book to a new location. But sometimes it doesn't.

(Related: Does anyone know of a PDF viewer that doesn't lock the stupid files when they're open?)
