Step #1: Move it yourself. It's a dockable widget. Move it to another dock area. Stack it/tab it with another. Go crazy. They're all yours to adjust however you want. They'll stay where you put them from instance to instance--through uninstalls and updates. And you're talking about the Book Browser, by the way. The TOC widget is already on the right by default (and in your picture). It's Book Browser that defaults to the left on new installs. The names of the widgets are in their titlebars. Using them correctly will go a long way toward being able to better help you in the future.

Step #2 is a no for me. There's no need to change a default shortcut when you can change it to whatever you want and have it stay that way (through uninstalls/updates). The defaults are for new installations only.

As far as #3 goes. The "middle panel" is the central widget. The only thing that will ever occupy that space is Code View. It is the central widget. Everything else will either be a dock widget or a separate dialog. It's not possible to "embed" the TOC editor (or any other widget/dialog in the central panel).

Step #4 is a "no" for me as well, I'm afraid. The ability to create new blank files already exists in the Book Browser context menu where it belongs. The editing of a TOC is not the place for new file creation, IMO.
