I've actually seen an ePub from Gutenberg that passed the then current version of epubcheck and it caused problems. It was an unnecessary CSS class that did it.

Smashwords can be awful because of the way some people have no clue how to use Word.

Archive.org is rubbish because it's automated scan/OCR with no corrections.

Amazon can be awful because whoever is making a lot of the Kindle Unlimited eBooks are made by the author who has no clue how to make eBooks.
