It will mark any items that are fixable; items with a expansion triangle next to them. If there aren't any such items then nothing will be marked. The button isn't disabled if there aren't any such items.

To test this, go to any folder containing a book in the calibre library that has a cover (cover,jpg) and rename that file to coverrrrr.jpg, then run check library. The cover section will now show a triangle and the "Mark all for fix" will check the box next to that line. I suggest not actually running the fix, instead re-renaming the file back to cover.jpg.
