Take a look at the KF8 file in the Calibre editor. You'll then be able to see why you didn't get an NCX ToC built.

Without seeing the code, there's no way to know why there was no proper ToC generated. Post an example of the Chapter header and the CSS that goes with it.

Is this eBook a single HTML file? Is it multiple HTML files? Is the code a mess?
