I would have suggested moving your question to the KFX Output plugin thread—check the Dealing with Conversion errors spoiler in the first message—and including more information. Include plugin output logs, version of calibre and the kfx plugin, version of Kindle Preview3, source of the epub (commercial, public domain, home brewed), etc. Please remember to wrap logs in spoiler tags.
