That's probably part of your current book's stylesheet and/or code.
CSS is not my forte, but you can check the code from within KOReader (there's a show HTML button in the highlight menu), and then, depending on how it's implemented, you might be able to hide it via a custom style tweak.

And, for future reference (ha!), the ref. page thing is an ePub 3 feature that was implemented somewhat recently, so you probably wouldn't have seen it often.
