Yep, that's the problem! It just started and only seems to affect certain books. Of several I've side loaded in the last few days, only this one looses the cover.
If you put the Kindle in Airplane Mode, the cover stays. Within a few seconds of re-connecting to Wi-Fi, the cover vanishes. What a pain.
