"Bad" as in "misbehaving". Basically books with errors in them, or possibly overly complex coding. Exactly what problems cause this, I don't think anyone knows. But, people have been able to directly connect particular books with the problem. And then cleaned up the code and the problem went away.

In general, if the error check in the calibre editor doesn't show problems, then you won't see a problem. For complexity, it is more a matter of how big the stylesheet and what the code looks like. There are probably some other things such as size of the internal files and use of embedded fonts.
