If original djvu file has already got TOC bookmarks (or if we can use some djvu tool to create bookmarked TOC), we can then use HandyOutliner app for copying bookmarks from original djvu into the converted pdf; as well as from original pdf into converted pdf or from txt file containing bookmarks.
