Do any of these failures happen without the use of "open with", ie.completely separating the two programs environments?
I'm using KiwiDude's Open with plugin to open the epub from within calibre.

I've seen this error opening an epub directly on a few occasions. Just this time, it happened consistently when I was opening two newly added epubs. The first one stopped showing the issue after I used the Restructure Epub to Sigil Norm tool, the other one when I modified the epub without the restructure. In both cases, I was cleaning up the drop caps since on my wife's Libra, the drop caps used a cursive font/CSS which took about 5 lines.

More a cosmetic issue since clicking on close in the popup opens the epub.
