Based on Tex2002ans's comments above .. we just added two more buttons to the AddExisting... image replacement dialog, "YesToAll" and "NoToAll".
This should make adding existing xhtml files with lots of duplicate image names you do not want easier, as well as adding lots of images to replace those in an existing epub.
The original bug report was from 2015 so this only took us 5 years!
This fix is now in master and will appear in the next release.
