My problem: KoboUtilities doesn't appear no more on Calibre.
I did update the extension, suppress and install it again, no change.
I process update again on the Kobo (.tgz), same.

What happen before: I've just bought a new Clara HD, before I had a Aura ed.2. I was able to install everything on the Clara, with no problem related to KoboUtilites. - Another thing I did is to put on "devmode" on the Kobo (for inverted text, but I tried widget option and then deactivate "devmode".

System: OSX 10.11 (El Capitan) - max system on my computer - so with Calibre 3.48
You seem to be mixing two things up. One is the Kobo Utilities plugin in calibre, and the other is things done on the Kobo device.

The mention of a ".tgz" is probably referring to the "KoboRoot.tgz" used to update the firmware. This happens on the device and has no direct impact to calibre or the Kobo Utilities plugin. The same with using the developers mode on the device. Nothing about that will affect how calibre or the Kobo Utilities plugin will interact with the device.

There should be no reason the Kobo Utilities plugin will not run with calibre 3.48. I have not changed the minimum supported calibre version for a very long time. In fact, it should work with 1.48, but, I don't think I have tested this recently.

I have been updating the plugin to support Python 3. It should be backwardly compatible. But, I might have an error. I have been testing the changes with 3.x, but, no were near as thoroughly as with the current release version. It could be a Mac thing as I don't have one to test with.

But, what does "doesn't appear no more on Calibre" mean? Does it mean that the plugin is not listed in the Plugins page of the preferences? And that installing the plugin appears to work, but, isn't in the list when you restart calibre? If so, please run calibre in debug mode and post the log. That should show the error when calibre is initialising the plugin.

Another possibility is that the toolbar button isn't showing. If so, you need to check that it is still on a toolbar using the preferences. When install the plugin, it defaults to being on "The main toolbar when a device is connected". But, you can add it to other toolbars yourself. If it not on the toolbar, but is in the "Available actions" list, you can add it to the desired toolbar.

If you mean something else, then can you explain in detail what is happening. Then I can look at what is wrong.
