Actually, I wouldn't try to convert this at all. If it's an AZW4 (aka 'Print Replica'), as it looks like to be, the OP would be better off using the KindleUnpack plugin to extract the PDF file.

And what a pain in the posterior converting PDF files to anything else is, we all know by now...
