The only reason I can think of for the screen to flip when your putting it to sleep is if it's getting tilted upside down right at the time it's going to sleep. (or the motion at the time makes the accelorometer *think* it's upside down.)
For me, I tried everything above. I use a sleep cover, but tried using the on/off button to see if that made a difference. Nope. I tried holding the Forma vertically when closing the cover to see if that made any difference. Nope. I tried holding the Forma vertically when opening the cover. Nope, nope, nope. I use a stand when reading, so the Forma was almost always vertical when it was on. Nope. Nothing made any difference except getting a newer Forma, which does not have this problem.
