It depends on the test. In the first post, I can see for tests that mention covers. Two will look in the books and one is looking in the library folders. The fourth could be either, but I think it is looking in the library.

So I have a few entries where details/cover browser shows nothing. I remember these are entries that used to show a cover before my last windows format.

These same entries also have cover files in their respective folders. And when I open the epubs, covers are there. And the editor confirms it - the cover I see when I open the epub was previously generated by calibre! It's not hassle to fix but trying to pinpoint where / how calibre forgot, why it isn't displaying the covers in the gui.
I don't think any of the options are checking that the cover is valid. One is checking for size, so it is possible that that this might pickup invalid files. But, it won't pickup the wrong cover. I don't know if any of the library maintenance options would help.

If you are sure the books have valid covers in them, then you use Bulk Metadata edit to extract the covers from the books. Select the books in question, click the edit metadata button and in the "Change cover" box, choose "Set from e-book files". Press OK and calibre will go through the books and either extract the cover or generate one. You could do it for all your books if you are daring enough.
