But I think you refer to the one shown in Details / cover browser.
(the first on the QC check list and again in the Missing check?). That is easy: There is no cover.jpg in the title folder (or it was not added USING calibre)  it might be in the book, but was not properly identified, thus failing to extract when adding to Calibre . View the book and repair it if needed, then use the 'cover icon on the left side of the formats box to extract a copy for the library view

Thanks. So this plugin is only checking the folders, I'll have to check manually in my specific case.
Edit: For what's below, I PARTIALLY figured out the problem. Somehow the cover.jpg in the folders got corrupted. The epubs are fine (I hope) but naturally the details/browser will fail and show a blank and QC wouldn't flag them because a cover.jpg exists there. For other entries, its displaying pictures from my photo albums or from other books. xD
Something else is going really wonky with my DB.
So I have a few entries where details/cover browser shows nothing. I remember these are entries that used to show a cover before my last windows format.
These same entries also have cover files in their respective folders. And when I open the epubs, covers are there. And the editor confirms it - the cover I see when I open the epub was previously generated by calibre! It's not hassle to fix but trying to pinpoint where / how calibre forgot, why it isn't displaying the covers in the gui.
