He isn't doing anything wrong. When adding new books calibre will always flag books with the same title for manual review.

If you use a separate intake library for new books you can avoid the duplicate on add because there won't be any books in your intake library and as theducks states you can go to Preferences: Adding Books: Adding Actions to configure it to ignore title duplicates when copying from one library to another.
