Your debug log shows that the cover image URL was somehow discarded between being reported and being fetched. I don't know what might have caused that; it shouldn't happen and it didn't for me.

I suggest that you re-install the FFF plugin and then try again. If you still get the same error, let me know and I'll make a version that has extra debug output to try and track it down better.
