Enter the first few items from your CSV list into Conversion S&R list, then Save the CSR file. Hopefully you'll then be able to work out a way to create a CSR file from your CSV.

If the ebook files in question are EPUBs, another possibility might be to use the Sigil ePubTidyTool plugin.
