I re-started in Safe Mode. Unfortunately, the problem persists.

I also tried it with F&R closed, and that makes no difference. Problem occurs with F&R open or closed.

Another thing I noticed as I play around with this bug is that the leftover dialog boxes accumulate if you keep repeating the process (they could be one on top of each other, so at first it appears that only the last dialog box persists, but if you move it, you find another one behind it). I did the following:
  • open epub1, select single file, do Save As, close window with red close button — all is well
  • open epub2, dialog box from epub1 reappears (could be hiding behind window), select single file, do Save As, close window with red close button — leftover dialog box1 persists
  • open epub3, dialog box from epub2 reappears, select single file, do Save As, close window with red close button — leftover dialog box1 and box2 persists (they could be on top of each other, so move box2 to see if box1 is behind it)
  • open epub4, dialog box from epub3 reappears, select single file, do Save As, close window with red close button — leftover dialog box1, box2, box3 persists (they could be on top of each other, so move them around) — at this point I have 3 leftover dialog boxes on my desktop! etc. and the only way to get rid of them is to Quit (command-Q or menubar-Quit)
