Copy and paste (text and images separately) from PDF to wordprocessor. Style headings and images.
Save As docx and import to Calibre.

It's not a Calibre problem. PDFs are only meant as Proof for Paper or a file to print on paper WYSIWYG. They are not a useful content source.
