A permissions problem wouldn't cause an I/O error message, and it wouldn't cause the USB D: drive to work at all times when not being used by Calibre. In subsequent posts you said other programs also had issues. I assumed you already power cycled the computer and the drive. If you still haven't tried running as administrator do it and report back your findings. Sounds like the calibre library may be read only. Don't assume how any program will report a permissions issue error. I/O error is as good as any when unable to write to a drive.
