Okay, I thought you said the problem only happens when using Calibre. Do you know how to check the permissions (under Properties/Security)? Make sure that Everyone or your user name or Administrators has access to everything, but I don't think that's gonna solve the problem, since you have no problem with some of the extensions.

Did you try the experiments I suggested with different USB drives and USB ports?
