A permissions problem wouldn't cause an I/O error message, and it wouldn't cause the USB D: drive to work at all times when not being used by Calibre. This error is only being generated when using Calibre. Also, whether being run as an Administrator or not wouldn't make a difference in the problem that results in an I/O error.
