Are you using paginated mode with multiple columns? If that's the case, this might be caused by the viewer default behavior (at least was default until 4.0).
When you click on the margins or use the mouse wheel to change page, the viewer change only ONE PAGE (i.e. one column). The second column is moved to the first column, and a new one is placed where the second was. So, the first column will always be repeated.
Starting with version 4.1, the viewer has an option (Scrolling Behavior) to change this to SCREEN FULLS, instead of PAGES.

Just noticed that using the ARROW KEYS still changes only the page. The options available are only applied to the mouse wheel and clicking on margins. Can you make ARROW KEYS behave like the mouse wheel?
