I stumbled over that myself. jackie_ws comment is just a little bit misleading. Naming the file <smth>_custom.yaml does nothing by itself, in fact, you can name the file anything you want. The important part, as geek1011 says, is adding a line to the patches section in kobopatch.yaml, where you map your file to the file to patch.
