Thanks for the suggestion. Now the background image doesn't work on the first 2 pages of any file but works fine the remaining pages of the file. It also doesn't work in flow mode. That said, duplicating both html and body does provide the background image on every page in flow and page modes.
My remaining issue still stands strong.
Issue 2: When I set the margins in viewer preferences instead of just setting the margin of the text it also shrinks the background image. Previously this wasn't the case. No matter what I have tried the text still rides along the edge of the image, which obscures the letters along the edge of the pages.
Update: Using P margin !important works. Thanks again for your input.
