An old NOOK wrangler here. I learned by experience that the NOOK will only create a thumbnail for a book if the cover image in the epub file is actually named "cover." (cover.jpg or cover.jpeg both work)

So just pop the book open in Sigil, change the name of the cover image, and re-copy it onto your NOOK.
