I do this by configuring my Kobo calibre driver. (KoboTouchExtended for me, possibly KoboTouch for you).

At the bottom of the 'File formats' tab there is a 'Save template' box. Mine reads:

Books/{author_sort} - {title}
I don't recall whether calibre will create the Books subdirectory, if it doesn't already exist, the first time you send-to-device. If it doesn't I'm sure you know how to create a Books directory in the Kobo root directory

I've used Books for mine but it can be anything you want.
