I've seen that in at least as long ago as 4.15.x too on my original H2O. I just check in the Annotations, after bookmarking page, that it's really gone (or not).
It's annoying, but not hugely annoying.
Just updated my Original H2O. I do like being able to have chapter heading. I don't need the name of book in footer, so I turned that and progress bar off as the chapter heading is set to % (book progress actually in an epub).
I do think this is slight improvement in reading GUI, though bookmarks and annotations/highlights ought to be separate, one tap less to annotations?
