The problem is that I don't know what you mean by "hard reset". Some people think it is pressing the pin-hole reset button. If that is what you mean, all it really does is restart the device. If by "hard reset" you mean a "factory reset" using either the option in the Device information page of the settings or using the buttons, then that is what I meant.

Formatting the visible partition when the device is connected to Windows is safe. It will wipe the partition, format it for FAT32 and when you disconnect, the device will detect nothing is there and take you through the setup steps. But, if you have done a factory reset, the partition will have been formatted as part of that. If that is the case and you are seeing this problem, it could be that the internal SD card is failing. Replacing it and restoring an image to the card will probably solve the problem.
