How did you transfer the book to the device?
Calibre does NOT automatically update the Library copy, but embeds the current metadata (inc the cover) as it sends the book (send to Device, Copy to Folder).
Use the OS to transfer and you bypass this process.

OTOH there are a number of tools and plugins the WILL update the Library copy. Embed Metadata, Polish and Modify EPUB (a PI)
