Nothing in the last version specifically changed how covers are handled. By default, FFF will not generate a cover (you have to turn it on), but FFF will include a title page with title, author, and several other metadata items by default.
It sounds like you had covers on and title page off, but now covers aren't generating.
How old was the version you were using? Of both FFF and Calibre.
Because it sounds like you're talking about Calibre's generated covers from the early days, like so:
Calibre stopped generating covers like that a looong time ago. And along the way, FFF's settings for generating covers changed also. There's a tab for "Calibre Cover" in FFF's settings dialog with a host of options.
