Plugins can look for icons on calibre resources folder (on Windows, generally found on 'C:\user\AppData\Roaming\calibre\resources\images' ).
So, its up to the Theme designer to create icons for plugins he wants to support. This theme, for instance, have icons for: Annotations, Count Pages, EpubMerge, EpubSplit, KindleUnpack, and many others.
