I don't have a separate TOC file. I removed the Contents page in Word before pasting it into Sigil as I was advised to do so as eBooks don't have page numbers. I created a TOC in Sigil via the Tools>Create Table of Contents, doing for both ePub and mobi.

I have now set the title page and other front matter headings in Heading 1, so at least they show up in the TOC and before the chapters. But it might still be good to have the TOC after the front matter.
