Question: Is helper+ saying "no to OTA updates" enough to prevent them from happening again? 2) i verified the update.bin.tmp.partial directory is there... not sure what else I can check.

That reads as if you are reporting a use case (model/(prior)firmware version) which can over-ride the blocking directory.
Please provide the following so that other people can duplicate the failure:
What firmware version was the device running prior to the OTA update to 5.11.1 (it is the firmware version that was running, not the firmware version that is be installed that needs to be known).
Would you please provide either an image or a screen capture of the file and directory layout seen at the top level of the visible USB mass storage.
