There was a bug with this at one time. It only looked at the file names and didn't consider the directories they were in. I remember constructing a file to demonstrate it and reporting it to Kobo. It was fixed almost immediately. And I retest as part of this discussion in case something had changed. And I couldn't remember what happened if there were images in the root directory of the file as well as subdirectories.
