If you had opened the Metadata editor: Selected the format on the right: then click the brown book Icon (gets cover from format): OK
That forces a fresh cover to be used for the library. there is also an Icon for metadata.
You discribed this way and it worked instantly:
Edit metadata->edit metadata individually->click on 'epub' icon in the top right corner->click on blue book icon left of it->click 'ok'.
Now there is a cover in the library and also in the book details window.

Your old book did not have the cover properly defined. That requires 2 things to be flagged (semantics) in the OPF.
1)the image file (the name IS NOT the flag)
2)the (x)HTML file that uses the image AS the cover. (this file can NOT also be flagged as something else. Covers are seperate)

Using the Add cover tool does the flagging.
This did not work (at least for me). 'Add cover' does not create cover.jpg in the folder, where the epub and opf files are. Weird.

But there is a complicated work around (at least for my case)
When I open 'Edit book' in a book with no cover in the library, i scroll on the left panel to the image with the tiny blue cover icon left to it. Then I open the context menu of the file and choose 'Export NAME_OF_FILE.jpg'. Next I look for the folder, where book's epub and opf are located and save the image as cover.jpg. Now do I close Calbre and when I open it again -voila- the cover is shown in the library.
