I've seen that happen through the years with various firmware versions on my Kobo Glo and now the Glo HD. I used to mark all books I was reading with annotations for edits to fix the errors and typos when I was done so I used to notice it a lot. But I got tired of spending time fixing someone else's mess when I could be reading more, so I rarely use annotations now, so the bug doesn't bother me anymore.

But it's the firmware, not your device.
