Displaying headers/footers is now part of the standard firmware. It looks like you may need to revisit your Reading Settings. This can be accessed via the cog-wheel icon in bottom menu when inside a book or via Settings>Reading settings from the main menu at top-left.

You should have 2 checkboxes there labelled 'Show title' and 'Show footer'. From your photo it looks like you may have the ticks the wrong way round.
